Acute Rehabilitation Unit

We're equipped with a 62-bed Acute Rehabilitation Unit offering comprehensive intense inpatient services that typically include three hours of therapy a day for at least five days a week. Our goal is to return all individuals to the highest level of functional independence. Our in-house physiatrist work collaboratively with nurses, care coordinators, occupational therapists and physical therapists to develop a rehabilitation program that's right for you.

Acute Rehabilitation Admission Criteria

In order to be admitted to the Acute Rehabilitation Unit, the patient must meet all the following requirements: 

• Need at least two forms of therapy in an inpatient setting 
• Be able to participate in three or more hours of therapy each day for at least five days a week
• Be medically stable
• Have potential to improve functional or achieve independence
• Require ongoing medical management and nursing care
• Be willing to follow the discharge plan developed jointly by the interdisciplinary team with the patient

Transitional Sub-Acute Unit

A skilled team of board-certified physicians and health professionals has been selected, trained and certified unlike any other post-acute staff to bring together the best and the brightest support. Our team includes an internal medicine physician, nurses, care coordinators, occupational therapist, physician therapist, dietitians, and as needed, speech therapists that will develop an individualized program for treatment. 

Transitional Sub-Acute Admission Criteria

To be admitted to the Transitional Sub-Acute Unit, the patient must meet all of the following requirements:

• Need daily skilled nursing care and/or specialized therapy
• Have had a prior acute hospitalization of 72 hours or more (unless waived by payor source)
• Be medically stable
• Require inpatient services, rather than home health or outpatient services
• Be willing to follow the discharge plan developed by the interdisciplinary team with the patient

Brain Injury Unit

Our 11-bed secured rehabilitation unit provides specialized care for patients with traumatic and acquired brain injuries. The Brain Injury Unit team, is comprised of the area's finest physicians and neuropsychologist who maintain special certification and training in brain injury, stroke, spinal cord injury, and musculoskeletal conditions. Many members of our nursing and therapy staff have earned advanced certification in the rehabilitation and neuroscience field – a unique attribute of our program.

Brain Injury Unit Admission Criteria

A minimum of three hours of therapy per day, five days per week. Therapy is also provided on Saturdays and Sundays according to your needs. Care coordinators work with you family to discuss progress and assist with plans for discharge. Your family or caregiver may be asked to participate in therapy sessions, family education, home evaluations or community re-entry activities.
